I am a developmentally disabled adult with a communication disorder. Ive repeatedly requested accommodations to ensure my examinations are a proper account of the symptoms and complications Im experiencing, but continue to be ignored by Evolve. The location of my psychiatric assessment was changed at the last minute and I was sent into the middle of major road construction, rough terrain and heavy equipment. I require a communication intermediary but am provided chaperones that have no idea what my needs are or why. Im experiencing extreme sensitivity to light, seeing floaters, major after image requiring me to cup my eyes. The pain at times is unbearable and always present. I become easily disoriented, confused, poor balance, extreme pain that requires me to lay down for a reset. Middlesex Mutual Insurance Company requires these assessments with providers that keep changing in order to determine whether I am eligible for further treatment. Issues with my back that for the most part had been worked out in my second treatment plan returned with a vengeance following my first Job Site Analysis with Evolve.
